Student-Athlete Checklist

The following forms need to be completed and returned to the Martin Luther College Athletic Office by all students who plan on competing in sports. These forms are required by Martin Luther College and NCAA policies before you are eligible to participate in your sport (including practice, conditioning, and try-outs).

Fall sport athletes need to return ALL forms by August 1st. Please print and complete the following forms, and mail together to:

First Year/Transfer Student Forms


(Forms due by August 1st)

Anyone who has not practiced or participated in Martin Luther College Athletics during the 2013-2014 academic year, or is new to MLC, needs to complete and return these forms by August 1st.

  1. Insurance/Emergency Contact Form**
  2. Photocopy of medical insurance card, front and back (student should keep a copy as well)
  3. Sickle Cell Trait Form^
  4. Injury & Concussion Acknowledgement Form (see link below for concussion information)
  5. Physical and Medical Health History Forms - Freshman and Transfer students need to complete these forms provided through our Health Services Department (these were sent in the mail in late February and are due by July 15th)

Returning Student Forms


(Forms due by August 1st)

Anyone who participated in intercollegiate sports previously at Martin Luther College needs to complete these forms and return them together to the Athletic Department by August 1st. (This also includes returning students who have never participated in athletics)

  1. Medical Health History Form
  2. Physical Exam Form (athlete needs a new physical every 2 years; generic forms will not be allowed)
  3. Sickle Cell Trait Form^
  4. Insurance/Emergency Contact Form**
  5. Photocopy of medical insurance card, front and back (student should keep a copy as well)
  6. Injury & Concussion Acknowledgement Form (see link below for concussion information)

Note: Certain prescription medications are classified as banned substances by the NCAA (some ADHD medications). It is your responsibility to report all medications to the athletic training staff. If you are taking prescription medications that are classified as banned substances, the NCAA requires a medical exemption form signed by your physician. See link below for more information.

** According to NCAA guidelines and Martin Luther College Policy, all students participating in intercollegiate athletics must provide evidence of their insurance which includes major medical coverage. No student-athlete will be allowed to compete or even practice/condition until such evidence is on file in the Athletic Office. If you are unsure of your coverage, you should contact your insurance company immediately.

^ Effective 8/1/2013: All NCAA Division III student-athletes must provide evidence of their sickle cell trait status or sign a waiver prior to participation. Please see the Sickle Cell Trait Link below.
